I didn’t use any perspective lines for this (Just played around with the skew tool) as this is a simple figure; but of course you should take the help of guides to have the right perspective for complex objects. It is an obvious process.

Once you have the perspective lines drawn, you can easily align any flat shape to it by using the Skew/Distort Transformation in Photoshop.

I like the outcome of this icon; it looks very realistic and still looks like an icon.
Do you draw the complete book by hand (freehand) or do you use some gidelines for the perspective?

I really like the result of this one, Supratim. It looks so solid that, for some reason, I feel like I want to pick up your icon and whack it against a table to hear how loud the bang is!
